Currently, gear progression is terrible REGARDLESS of whether or not you are a casual, hardcore, or in between the 2, usually called midcore. Though this might be an issue with item level as a progression system as a whole, but here's list of content that does NOT drop specific gear pieces that are NEEDED for gear progression;
*No Weapons: Level Cap Dungeons, Normal Mode Raids, Alliance Raids, Normal Mode Trials, Variant/Criterion Dungeons as whole, FATEs, Chaotic Mode Alliance Raid(unsure if more are going to be added in the future), The Hunt(used as catch-up mechanic for outdated tomestone gear), Grand Companies, PVP, Treasure Dungeons, and at least ONE Extreme Mode Trial per expansion.
*No Armor: Normal/Extreme Mode Trials, Variant/Criterion Dungeons, FATEs, The Hunt(same as weapons), Grand Companies, PVP, Treasure Dungeons, and Deep Dungeons.
*No Accessories: Normal Mode Trials, MOST Extreme Mode Trials with exception of at ONE per expansion(except Heavensward for some reason), Variant/Criterion Dungeons, Alliance Raids, FATEs, Chaotic Mode Alliance Raids(again, unsure if there will be more added in the future), The Hunt(same as weapons AND Armor), Grand Companies, PVP, Treasure Dungeons, and Deep Dungeons.
Additionally, there is no way to upgrade old outdated gear to a higher item level, that is MOSTLY on par with Savage Mode gear that ISN'T Tomestone gear(which is supposed to be replaced by the battlepass in the Evercold expansion but we will see how long THAT WILL LAST) and Level Cap Crafted gear, and the 1-2 guaranteed Materia Slots is outdated design with the existence of 5 guaranteed Materia slots that was introduced in Stormblood that the developers didn't make more of after a certain point in Shadowbringers.
It ALSO doesn't help with the fact tanks or healers that are avoiding Tenacity/Piety IN GENERAL like they are the plague do those stats not providing Direct Hit Rate+Skill/Spell Speed AT ALL, an issue that DPS do NOT have, and the developers are STILL TRYING TO FORCE PLAYERS TO USE THOSE TWO SUBSTATS WHEN THOSE TWO SUBSTATS ARE NOT WORTH GOING FOR OUTSIDE of casual players, who don't even know what or how to meld in the first place.
