Frequency: Always! Seemingly everytime "Cerberus breaks his chains!"

Class/Level: Level 50

In-game time: N/A
Steps:
1. Enter World of Darkness
2. Beat your way to the second last boss, Cerberus
3. Start the battle!
4. Midway through the battle, "Cerberus breaks his chains!"
5. Except he doesn't actually break them?
Please refer to the video! You can see that even though the 'breaking chains' animation plays, Cerberus' faces remain masked. This is confirmed to be happening in both PC and PS4 versions.
