From a lore perspective it is unreasonable to think that every single person on the Source is fit. In Ul'dah you'd expect Lolorito, [deceased], and Dewlala to be fat because they don't do any manual labor. The Lala at Costa Del Sol who just sits on a chair 24/7 frolicking at catgirls should, in any set of physical laws, be morbidly obese. But we had to get to the First to see any obesity (and coincidentally all of them are catgirls). So this leaves us with the following hypotheses:
1. Only Mi'qote can be fat. All other races have genetic codes that support a fast metabolism. Especially Lalafells.
2. Because of the Calamity happened recently everyone starved and got thinner, and haven't got enough time to get fat again. (In Eulmore the last "calamity" took place 100 years ago).
3. Fat-removing magic is common practice on the Source, and mages are hired to remove fat from wealthy patrons. Poor people who can't hire those mages have to do manual labor and are therefore fit anyways.
Interestingly Hrothgars and Roegadyns are also extremely muscular to a disproportionate extent. It is possible that working out is part of Hrothgar and Roegadyn culture.
