There seemed to have been a lot of people with my issue in the past, but this took me an entire day to figure out so I felt the need to share my results.

Issue: In windowed/borderless mode, my FPS is capped at 30, when my cap is set to 60. It becomes 28 or 29 if I cap it at 30, and when uncapped, the fps value will go much higher than 60, but I can tell the window's fps is still around 30 despite that high number.

Solution: I don't have enough evidence since I undid all the settings I changed yet the game remained at 60 fps, but I believe my issue is V-sync, because I think I can just barely notice some tearing if I shake my camera violently. Also the default is to "Use the 3D application setting," so it makes sense that if I forced it off, and reverted it back to use the application settings, the application had already set the option to off so it stays off.

How to change setting: Right click on your desktop and select NVIDIA Control Panel. Under 3D Settings, click Manage 3D settings. Go to Program Settings, select FFXIV (probably ffxiv_dx11.exe), and change the vertical sync setting to off.

Make sure to restart your computer after this before logging back in (as that's what cost me the most of the day by thinking restarting application was enough).

This may not be your solution, but I believe it was mine. Another common solution that seemed to work for some others is to go the the application properties and check the "Disable fullscreen optimizations" box under compatibility.
