Frequent, Seemingly Random Freezing Issues after DT Patch

Ever since the Dawntrail patch, I've been suffering from a consistent freezing issue which only seem to plague FFXIV.

Once it starts happening, which it happens at random, my game client (and only my game client) completely freezes up for about 20-40 seconds at a time. Every 5-10 minutes or so, it'll freeze again for the same duration. During this, I apparently get the disconnecting symbol/Pokeball. The only way to temporarily fix this is to shut down and start my PC back up.
I've already tried changing in-game graphics settings, including the new ones, but to no avail. My PC's internal temperatures are normal when these freezes occur as well, and I have done a clean install of the newest NVIDIA drivers. In addition, I tried disabling fullscreen optimization on my ffxiv_dx11 and disabling the Steam overlay (I play with the Steam version).

The one time it did crash, which is when I had tried changing from either borderless window to fullscreen (or vice versa, I forget which), I got the following error codes:

I was going to try reinstalling the game, but before I do that, I wanted to see if there was anything else I should try first, or if this info could otherwise narrow down what's going wrong. None of my other games are having this issue, nor does the rest of my computer freeze when the game does.
