Steps:
1. I took place in the final ranking event of the Firmament while in Phoenix. On the leaderboard site, you can see that my character is ranked #52 in Mining (Link) and #54 in Alchemy (Link). After the leaderboard season had ended, I redeemed the title from the mail (I still have the mail saved at the postmoogle).
2. Some time after the event, I ended up moving servers to Brynhildr in order to play with friends while they were getting back into the game. I didn't notice that the title had been deleted from my account until the 18th March.
3. Since then, I have moved back to Phoenix, but the title is still missing from my characters title list.
