This post referencing this twitter post made it clear that very very rarely, a NPC from a DRK quest in HW shows up in the final boss room to specifically one person. The actual NPC and its text are taken direct from the quest, but it's pretty clearly a Drakengard reference. Everyone wants to call it a bug. It's not. Yoko Taro is just a madman.
ACT has a Ultrascouter plugin that "scouts" nearly NPCs, allowing for things like TTS whenever a Hunt is nearby. But the way NPCs work is that any NPC from any quest step is always present in the info being sent to the client in this regard, just hidden. For example, this is what is shown near a specific spot in Amaurot when not on the quest step.
So when I got a beta version of the plugin and went to the final boss room in Nier, I saw this (raid member names redacted). The ID for the NPC matches what is shown in Garland Tools, as well. 1015046 (F7D06 in hex).
In the exact same way as other NPCs, the dragon is there, but shown only to one person in maybe one raid group in 1000, if that, given how little there has been posted about it. Unless we get confirmation it only shows if you're on the exact step of that DRK quest, there's no way there's an overlap between it showing and the real one showing. It's just very unlikely and random. And I have no idea either why or how they'd drop it in without meaning to. It's just an Occam's razor thing, it has to be intentional.
What I want to know is how Yoko Taro even knew this NPC existed to put in as a reference. It's just insane and very much a thing he'd do to put it in, though.
